HOME CARE - SOME RESPONSIBILITIES:
- Responsible for completion and submission of OASIS Assessments, Follow up visits, Recertification, Transfers, Resumption of Care, and Discharges in a timely manner.
- Maintains proficiency in clinical assessment skills.
- Medication/pain management
- Provide excellent communication efficiently to patient PCP and family as appropriate.
- Regularly communicates with the office after new patient Admissions and all patient status changes.
- Writes clinical/progress notes on the date of the visit and incorporates the same in the patient's record according to company policy.
SUPPLEMENTAL STAFFING (REHABS AND HOSPITALS):
As an RN you have the option to pick up shifts at one of our partnered facilities.
- PRN shifts as needed.
- Shifts vary 8, 12, or 16 hours depending on your availability.
Licensed Practical Nurse

REQUIREMENTS:
- Home Health Aide 75-HOUR Florida State Certificate. (CNAs who want to do Home Care, need the Home Health Aide Certificate)

HOME CARE - SOME RESPONSIBILITIES:
- Provides personal care to assigned patients under the direct supervision of a Registered Nurse and responsibilities may include:
- Assisting patients with or performing: Bathing (tub, shower, bed bath), Oral hygiene care including denture care, Shampoo/Hair care/ Nail care; cleaning & filing only, Dressing and undressing, Toilet activities including the use of bedpan, urinal, bedside & commode
- Positioning/turning.
- Assisting with ambulating and transfer activities including Transfers in/out of bed, to a chair, wheelchair, toilet, or bedside commode. Assistance with a walker, cane, crutches, or other ambulation assistance devices
- Assistance with nutritional and dietary needs including Meal preparation, serving meals, Feeding, and washing/drying dishes.
- Provide a safe and sanitary patient environment including Making the bed, changing linen as needed, dusting and a tidy room used by the patient, and washing the patient’s personal clothing.
- Writes clinical/progress notes on the date of the visit and incorporates the same in the patient's record according to company policy.

- Assist clients and their families with personal and environmental difficulties, which predispose them toward illness or interfere with obtaining maximum benefits from medical care. These range from counseling members of the client's family to assisting clients with admission to a nursing home.
